All staff and volunteers are to complete this form if you are planning to transport youth.
Included are the policies regarding transporting youth:
5.3.7. Transporting People in Vehicles:
a. shall only be done by a Registered YU Driver who is at least 25 years of age and who has submitted the following information in writing: name, driver's license number, year and make of vehicle, name of vehicle owner, license plate number, insurance company and policy number, liability limit, age and driving record. A YU Driver Registration Form is available from the People and Program Development Director.
b. shall only be done in a safe and road-worthy vehicle.
c. shall only be done with minors after signed consent has been obtained by his or her parent or guardian, typically on the trip or program permission form.
d. shall only be done with children or youth using a sign-in and sign-out form that records the name of the passenger, the times and her or his signature upon leaving. Passenger Sign sheets are available on the staff only section of the YFC Calgary website.
e. shall be done in a safe manner, obeying all traffic laws, including the use of seatbelts.
f. shall only be done when the vehicle is insured with a minimum of $2 million public liability; $3 million is recommended on personal vans used to transport youth.
g. any traffic or parking violations incurred are the responsibility of the driver and will not be paid by YFC.
